Once you’ve selected the product you want, you've read the fine print and you’re happy with our quote, you can apply for membership.
Join Online
This is the quickest and easiest method of applying for membership.
Join via Mail or Fax
If you prefer, you can download and print our application forms to complete at your leisure and send to us.
- Everyone needs to complete a Membership Application form.
- You also need to complete the Federal Government 30% rebate form, unless you do not wish to claim the rebate as a premium reduction.
- If you are transferring from another fund, you also need to complete this transfer form.
- If you want your contributions paid automatically, you need to also complete one of the following forms:
- If you have student dependants aged 21-24, you also need to complete the Student Declaration form.
